If you choose to join the scheme then you need to complete the application form and nomination form available on this page. Once you have joined the scheme you have the opportunity to add Additional Voluntary Contributions (AVCs) these can be made using the AVC form.

In the majority of cases a claim form will not be required when you undertake private treatment, which helps reduce inconvenience for you and ensure payments to hospitals/ specialists are made within ten working days
